Japan Airlines Flight 123
This is one of the last pictures recovered from Japan Airlines Flight 123, moments before it crashed. You can see that the oxygen masks are out for everyone. Only 4 people on board the flight managed to survive and the other 520, including the crew, were killed in the crash.
